How we create an individual SEND plan
The aim of our provision is to identify and assess special educational needs and disabilities (SEND) and to plan the provision needed for each pupil to achieve his or her best at St Matthew’s Primary School.
Some SEND pupils may need extra support to achieve their learning potential. If this is the case, then we will formulate an IEP (individual education plan) or an IBP (individual behavior plan) in which specific targets will be made to aid the inclusion and progression of your child. The class teacher will also speak to the child so that their views and what he or she wants to achieve on their learning journey through St Matthew’s Primary School form part of the plan. Parents are invited to liaise with teachers so that their views and input are also included and they can express the outcomes they wish for their child, this will usually be completed on parents evenings. Progress towards these targets will be communicated with the child and parents regularly. The main aim is for the teachers, support staff, parents and pupil to work together to achieve agreed outcomes.
Some SEND pupils may need support from professionals outside of the school setting to achieve a successful learning journey. If this is the case, then the agencies work alongside school to aid inclusive practices. Some of the agencies we work closely with are:
- School Nurse or specific nursing teams such as the diabetic nurses or dieticians
- Health visitors
- Educational Psychologists
- CAMHS (Child and adolescent mental health services)
- The Learning and Cognition Team
- The Physical Difficulties Team
- Visual/ Hearing/ Mutli-Sensory Impairment Teams
- The Autism Team
- The Emotional, Social, Behavioral Support Team
- Families First
- Speech and Language Team
- Downsyndrome Support
This will mean there will be more people at the planning meeting, but everybody will have a responsibility to help the young person be successful on their learning journey. These agencies may also observe or work with your child in the school setting but your permission will always be requested before an external party works with your child.
Using St Matthew’s IEP and IBP Plans, we are able to develop support systems to help families who have asked for our help because their child is failing to make good learning and/or social, emotional and behavioural progress. There are many reasons why this may be happening and we are able to offer advice or support to the family depending on their needs.
EDUCATION HEALTH CARE PLAN (EHCP)
Education Health Care Plans have replaced Statements of Educational Need. If a child’s needs fill in to the EHCP section of the progress grid or higher, the school may ask for an assessment to see if they qualify for an EHCP. We will ask you to meet with us to discuss your child for the, ‘all about me’ part of the document. If they receive an EHCP, they will receive extra funding to support their provision. EHCPs have a joined up approach to assessing, setting targets for and monitoring your child. The following sectors work together:
Special education provision, health provision and social care provision.
If a child has an Educational Needs or an Education, Health and Care Plan, then we provide the support detailed in the plan. Bradford Children’s Services SEN department will be responsible for the EHCP and reviews will be held at least once a year to ensure the pupil’s needs are being met.
In order to help your son/ daughter achieve their learning potential, we may ask your permission to access Early Help. A member of staff, who is known to you, will ask you to provide information about your child and your family and will then contact the Early Help Team. We will only include information that you tell the member of staff. This team can then support families who need additional support.
